Q2 Do students need to bring proof of disability status if they are disabled, or we take their verbal words?

It is a local decision. We want ensure we can track students with actual disabilities. Generally speaking, proof is not required if the disabilities are self-reported, or are identified through assessments they received.

Q3 For intake forms, is there a way that on the AEP site, agencies could share what they have? That way we could see what other agencies use for Intake/Registration forms.

The AEP Office can request intake forms via the newsletter and post what is received from consortia members on Web site.

Q23 42. I wanted to receive guidance on providing bus service (don't worry, I'm not asking about trying to buy a bus!) to students in our service area to get them to/from classes. Background: One of our key elementary site partners is going through a major construction project, so we've had to stop offering classes on that campus and it is too far for most of them who lack a car to walk. I would like to explore paying for a pick-up/drop-off for those students, which would be more convenient than taking a public bus route. Allowable? Guidelines?
